I understand and agree that: (a) there are inherent dangers associated with the participation in martial arts, which could result in consequences including, but not limited to, substantial bodily injury, death, illness, including, but not limited to, illness caused by communicable disease, and significant social, psychological, or economical losses, to name a few (collectively, “Adverse Consequences”);(b) Adverse Consequences may result from the action, inaction, or negligence of the participant or others including, but not limited to the RELEASED PARTIES, or inherent risks associated  with engaging in martial arts and any injury or damage that I may cause or suffer from my participation; alternatively, I agree to bear the costs of such injury or damage myself. (b) I am willing to assume the risk of any medical or physical condition that I/my child may have, if any, whether known or unknown, which might be exacerbated by participating in martial arts. (c) To the best of my knowledge, that I have no communicable disease, infection, illness, virus, or bacteria that might transfer to other participants. I assume any and all risk and responsibility for any and all actions, inaction, or negligence on the part of the RELEASED PARTIES. I nearby voluntarily release, forever discharge, and agree to defend, indemnify, and hold harmless RELEASED PARTIES from any and all claims, demands, and causes of actions, which are in any way connected with my participation in any Events sponsored or produced by Practical Martial Arts, Inc’s equipment and/or facilities and the equipment or activities of the Event’s sponsors, including Practical Martial Arts, Inc. This release, waiver, and indemnity agreement includes all claims which allege negligent acts or omissions of any RELEASED PARTIES. I understand that this means I will be responsible for the payment of any attorney’s fees incurred by the Released Parties in connection with any claim, demand, cause of action, or lawsuit.
